What is a Home Care Package?

A Home Care Package is government-funded support that helps older Australians live independently at home. It includes services like personal care, transport, cleaning, and health support — all tailored to your needs.

From November 2025, Home Care Packages will be replaced by the new Support at Home Program, which offers a simpler, more flexible approach to in-home care.

How to get started

    Assessment: Contact My Aged Care at 1800 200 422 or visit their website to arrange a free assessment by an Aged Care Assessment Team (ACAT) to determine your eligibility and care needs.

    Approval: Once approved, you will receive a letter confirming your eligibility and the level of Home Care Package you are entitled to.

    Choose a Provider: Contact Better Connected to discuss your needs and create a personalised care plan.

    Begin Services: Once your care plan is in place, our team will start providing the  tailored services you need to live comfortably and independently at home.
